Our people 

As a CPA, you’re an essential team member who partners with other Sound nurses, site leaders, and the hospitalist team to improve quality and efficiency in the care of hospitalized patients. The CPA will report to nursing leadership. You’ll improve patient outcomes by promoting health, preventing readmission, and addressing barriers to healthcare access before discharge. You’ll also work to improve the patient experience and optimize resource utilization. While you won’t provide direct patient care, you will interact directly with patients at the bedside through patient satisfaction and discharge preparedness conversations.

Our practice 

As a valuable member of the hospitalist team, your role involves identifying and confirming patients eligible for specific programs, collaborating with the onsite team to establish patient care goals, and coordinating daily priorities, including discharge planning in conjunction with clinical performance nurses. You will actively engage in patient and family education, providing information on post-acute options and community resources. Additionally, you’ll follow a structured process to schedule community provider post-discharge follow-ups for patients deemed high-risk for readmission. You’ll also complete bedside patient feedback rounding, collect data to identify process improvement opportunities, and actively participate in special projects as needed.

As a CPA, you’ll work 7 a.m. to 4 p.m., with a lunch break, Monday through Friday, in a fast-paced environment while maintaining excellent attention to detail and accuracy in all you do. This role requires a passion for improving patient outcomes and working in a collaborative team environment. As part of our practice, we’re looking for someone with:

  • The following certifications are accepted for this position: Certified Nursing Assistant or Certified Medical Assistant
  • An active and unrestricted license in the relevant state for this position.
  • Three-plus years of experience in hospital or post-acute care settings.
  • Proficiency in technology, including electronic medical systems and Microsoft Office.
