Acute Care – Hospital Medicine, Critical Care, Emergency Medicine
Transitional Care – Skilled nursing and rehabilitation centers and in-home care management
The Transitional Care Advanced Practice Provider role is at the forefront of healthcare reform. Sound Physicians is expecting to make a significant impact in the way patients are managed in the immediate post-discharge phase. Advanced Practice Providers working in tandem with hospitalists and PCPs, bridge the gap with providers in skilled nursing and rehabilitation centers. We believe that Transitional Care Services will play an important role in healthcare reform.
For clinicians who already have experience leading teams, Sound has tools to enhance these skills, with specific courses on change management, process improvement, and our newest module – Getting Stuff Done.
